Alpha blockers.
These drugs work through the autonomic (automatic) nervous system by blocking nerve receptors that are called alpha receptors. Alpha receptors normally promote constriction of the arterioles. Blocking constriction promotes dilation of vessels and lowers blood pressure as well as reducing the work of the heart in some situations. Alpha-blocking drugs also inhibit the actions of one of the adrenal hormones, norepinephrine, that raise blood pressure as part of the fight-or-flight response. Alpha blockers are usually prescribed along with other blood-pressure-lowering drugs, such as a betablocking drug and/or a diuretic. In general, alpha blockers are not as effective for initial therapy as some of the other blood-pressure-lowering medications. There are now several medications available that combine the effects of blocking both the beta and alpha receptors.

H 2 blockers
These drugs (cimetidine, ranitidine, famotidine, available intravenously and orally; and nizatidine available orally) are inhibitors of histamine at the H2 receptor, thus suppressing gastrin-stimulated acid secretion and reducing gastric juice volume. Histamine-mediated pepsin secretion is also decreased. Nizatidine, famotidine, cimetidine, and ranitidine are available without a prescription in the US. H2 blockers are well absorbed from the gastrointestinal tract, with onset of action 30 to 60 min after ingestion and peak effects at 1 to 2 hours. Duration of action is proportional to dose and ranges from 6 to 20 hours.
For duodenal ulcers, once daily oral administration of cimetidine 800 mg, ranitidine 300 mg, famotidine 40 mg, or nizatidine 300 mg given at bedtime or after dinner for 6 to 8 weeks is effective. Gastric ulcers may respond to the same regimen continued for 8 to 12 weeks. Children ≥ 40 kg may receive adult doses. Below that weight, the oral dosage is ranitidine 2 mg/kg for 12 hours and cimetidine 10 mg/kg for 12 hours. Cimetidine has minor antiandrogen effects expressed and, less commonly, erectile dysfunction with prolonged use. Mental status changes, diarrhea, rash, drug fever, myalgias, and hypotension after rapid IV administration have been reported with all H2 blockers, generally in < 1% of treated patients.
